Students applying to the Middlesex Transition Academy (MTA) must be completing Grade 12 and must have an identified disability.
Prior to applying, parents/guardians should contact the program to schedule a visit and tour.
If parents/guardians and students are interested in applying to the Academy after visiting the program, an application can be filled out and downloaded from our website (see link below).
After the application has been submitted, the Academy will schedule an interview with the student.
After review of the application, a letter of acceptance will be sent to the student and they will be invited to visit the program to meet the students and staff.
The Academy's Program Coordinator will attend the student’s Planning and Placement Team meeting to review and answer questions about the Academy’s program with sending school district personnel, parents and student.
